Will, I have an PRE Andalusian mare who went through knee surgery to find she has no cartilage. After several months the surgeon said she was ready to be started and said I should NOT lunge her but get back onto her back and do a lot of walk and a bit of trotting. After 3 days she was lame again. My surgeon came to the conclusion she would no longer be an competitive horse but only good for trail riding. She has been out of work for over a year now. I want to bring her back to some sort of work whether it be light riding or trail riding. She has gained a lot of weight and lost a ton of muscle. She is too hot to get on her back, so despite what the surgeon said I am tempted to do the lunging with side reins as seen above (the horse coming back to work). I am wanting to try very large circles (the surgeon is concerned about putting pressure on the knee from circles), and I believe the side reins will make her a little more submissive to the lunge because without them, she will go crazy. What are your thoughts?

I would hand walk the horse with a bridal on for control until the horse calms down, which could take some time. Your vet is right, don’t push the lunging as the horse, even with the side reins, could explode and cause even greater damage to the horse.
